There are some people who use photoshop and "slice" it, producing an automatically generated table filled with image sections sliced by photoshop. This is never recommended and often produces errors with the way that it displays.
There are then some people (like myself) who like to code everything by hand-typing it, as I am actually doing right now for this post. I use a program called Coda by Panic, INC
In between these two methods is the "Design Friendly Method" which uses photoshop for the design and layout, but incorporates the use of AP div tags in Dreamweaver.
